The tool does the following things:
- displays a set of symptoms
- let's users select the symptoms they identify with
- let's users submit their symptoms
A mockup is attached. The best way to understand the interaction is to study the mockup. A list of symptoms is also attached.
We suspect that the best way to get users to identify the symptoms that matter to them is to move a slider back and forth to highlight a set of symptoms and then place that slider when they see the set of symptoms that best match their situation. That's the interaction represented in the mockup. If you think you can come up with something better, then go for it.
Please deliver a static document that represents what users would see when they arrive at the tool -- a styled version of the mockup is totally appropriate but if you have improvements to the design or want to move the pieces around that is okay too.
The style and color palette should be directionally consistent with the style of http://www.caring.com.
